At Cook Children's, taking care of kids means including the whole family. We take the time to listen to you and your child, answer questions and help you navigate the "perils" of parenthood.
We're here to help you with well-child visits, illnesses, behavior problems, learning disorders, or just to answer the questions that all parents have at one time or another. Together we'll get your kids on the path to good health and help you raise happy, well-adjusted children.

Our doctors are specialists in the health care of infants, children and adolescents. Our team includes board-certified pediatricians.
Dr. Dao received his undergraduate degree with Cum Laude honors from Texas Christian University in Fort Worth. He earned his medical degree from the University of North Texas Health Science Center in Houston, Texas in 2005 and completed his residency at University of Arkansas Medical Sciences/Children's Hospital in Fayetteville, Ark. He is board-certified in pediatrics and has been with Cook Children's Physician Network since 2008. Outside of the office, his interests include black and white photography, cycling, tennis, snow skiing and volleyball.

Dr. Harris completed both her medical degree and residency through Texas Tech University in Lubbock, Texas. She completed medical degree at the Texas Tech University School of Medicine, while finishing her pediatric residency through the Texas Tech University Health Sciences Center. Dr. Harris is board certified in pediatrics.
Dr. Raine earned his medical degree from Duke University Medical School in Durham, North Carolina. He completed his pediatric residency at Yale-New Haven Hospital in New Haven, Connecticut. Dr. Raine is board-certified in pediatrics.

Just print them out, fill in the information, sign them and bring the completed forms with you when you come in for your appointment.
