Check-ups and physicals
Cook Children's Emergency Department is supported by one of the country's largest pediatric transport programs. More than 2,000 children are transported to Cook Children's Medical Center annually, most for critical care.
Our doctors are specialists in the health care of infants, children and adolescents. Our team includes board certified pediatricians.
Dr. Jenica Rose-Stine joined the Cook Children's Willow Park practice in 2008. She is a two-time president of the Fort Worth Pediatrics Society and holds committee appointments at Cook Children's. She is enthusiastic about several areas of pediatrics including development, safety and prevention, and nutrition and she thoroughly enjoys her interactions with first-time parents and their newborns. "I can truly say I have my dream job. I work and live in a wonderful community, where I interact with my friends' and neighbors' children. Watching these kids grow up has been a joy, and I hope someday I will take care of those patient's kids! Now that I have kids of my own, I really enjoy the conversations with my patient families as we raise our kids together. I think of it as an on-going conversation we will be having for the next eighteen years."Dr. Rose-Stine's husband, Matthew Stine, is a family physician in Willow Park. The couple met in college and married in medical school. They have one daughter and one son. Dr. Rose-Stine enjoys being outdoors, reading, running, scrapbooking and chocolate.

Dr. Diane Arnaout joined the Cook Children's Willow Park practice in 2011. Dr. Arnaout was born and raised in the Dallas-Fort Worth area. She served as a leader on the medical education committees during her internship and residency in pediatrics at the University of Texas Health Science Center in the Texas Medical Center at Houston, TX.
After working as a pediatrician in Houston for a year, she and her husband decided it was important to be near family, and happily returned home to DFW. Dr. Arnaout is an active member of the American Academy of Pediatrics, the Texas Pediatric Society, the Texas Medical Association, and the Fort Worth Pediatric Society. She loves all aspects of her job, and takes special interest in asthma care and control, ADHD and learning disabilities, nutrition at all ages and the healthcare of teenagers.
She and her husband Drew live in Fort Worth, where he works as an attorney. Her favorite pastimes include running, reading, cooking all sorts of things, and photography (especially children's photography). Dr. Diane and her husband are expecting their first child this fall.

Just print them out, fill in the information, sign them and bring the completed forms with you when you come in for your appointment.
