Nursing and rehabilitative services
Services include initial discharge planning, patient assessment, home care instruction and ongoing treatments.
Each patient's care is managed by a Cook Children’s Home Health care coordinator who is a registered nurse that:
- Organizes or attends pre-discharge conferences, if needed.
- Works closely with case managers, discharge planners and doctors prior to a patient’s hospital discharge.
- Supervises individual patient care plans.
- Coordinates transition from hospital to home.
- Informs doctors of progress.
- Provides age-appropriate disease or condition-related information to the patient and family members.
Nurses, social workers, physical, occupational and speech therapists are trained in pediatric home care. Caregivers work with patients daily or weekly, and can anticipate patients’ needs and support. They also provide timely and accurate communication back to physicians.
Patients may call a pediatric home care nurse at any time, 24 hours a day.
Physician referrals
To make a referral, call 682-885-6294 or 800-747-8242. A Cook Children’s Home Health representative will assist you. Referrals may be made 24 hours a day, seven days a week. Office hours are from 8:30 a.m. to 5:30 p.m., Monday through Friday. After hours, an answering service will connect referring physicians or clinicians to the on-call nurse.
