Cook Children's Home Health

Sometimes kids need long-term care, or will heal better in a familiar setting. We provide quality, patient-centered care in the comfort of home. Led by a clinical team with vast experience in pediatrics, our visiting nurses have a minimum of five years of pediatric experience and our skilled visiting nurses are PALS certified.

To make a referral to the Fort Worth or Dallas locations, call 800-747-8242. A Cook Children's Home Health representative will assist you.

Home Health Service Area

Our service area includes patients within a 70-mile radius of our downtown Fort Worth offices. Medical equipment, pharmaceuticals and supplies can be shipped to locations around Texas and often to surrounding states. Please call with questions if you have a patient outside our clinical service area.

If your child needs to visit one of our locations to receive an infusion, be fitted for an orthotic and prosthetic device or pick up a piece of durable medical equipment, like a wheel chair, here is a map to help you.

  • All skilled visiting nurse staff are required to be PALS certified.
  • Most visiting nurses have a minimum of five years of pediatric experience.
  • Clinical leadership has more than 30 years of combined pediatric experience.
  • Most rehab therapists have a minimum of 15 years of pediatric experience.
  • Re-hospitalization rate over the last three years is at 5.1 percent, compared against a national average of approximately 19 percent.
