Schedule an appointment or refer a patient
- Referring physicians may send patients directly to Cook Children's Infectious Disease for inpatient or outpatient evaluation.
- Patients can make an appointment and physicians may refer a patient by calling our offices at: 682-885-1485
- Routine office visits are by appointment only. Please call as soon as possible if you must cancel or reschedule your appointment.
- New patients, for your convenience, please have your insurance information ready when you call.

Clinic hours
Our clinic hours are 8 a.m. to 5 p.m., Monday through Friday. If you need to talk with one of our physicians, please call 682-885-1485. After hours, please call 682-885-4000 and request to speak with the infectious disease doctor on call.
Referrals
Many insurance plans require a referral for services. Please check with your insurance plan about the need for a referral. Your primary care doctor will authorize the referral prior to receiving specialty services.
Fees, payments and insurance
Cook Children's Medical Center accepts many different insurance plans. Please call 682-885-1485 to make sure that we accept your insurance plan. Cook Children's Medical Center charges for the use of facilities, as well as for any tests and procedures. Doctors' charges are billed separately. Cook Children's Medical Center and the doctors' billing services will file the patient's medical claims.
Fees, insurance co-payment and deductibles are due at each visit. Please bring insurance identification to every appointment and let us know when there are changes in insurance coverage, address, employment or phone numbers.
