The following checklist and resources make your decision-making process easier and your visit go more smoothly.
It is important that we have your child's rehab referral from your doctor when you come for a new appointment. Have your doctor fax it in to: 817-347-2985, or bring it with you.
It is strongly recommended that you check with your insurance plan on your benefits. You will also need to bring your driver's license and the patient's insurance card.
It is always beneficial to bring copies of previously taken radiographs, MRI or CT scans. The images will be reviewed by the therapist and possibly compared to radiographs taken in the office during your visit.
In addition, patients should bring any existing braces or orthotics they have – especially if they feel adjusting or replacing is necessary. This allows the therapist to assess the child's needs more accurately. Bracing may include, but is not limited to, ankle foot orthosis (AFOs), knee immobilizers, ankle supports, special shoes or inserts, wrist braces, slings and prosthetic devices.
You should receive an automated confirmation/reminder call 48 hours before your scheduled appointment. The message may include such details as the patient's name, the date/time of the appointment, and the location.
Appointments typically last for one hour. We ask that you try to arrive fifteen minutes early. This assures that if we have any questions or need to update your information, we can do so without delaying your appointment time.

Wear comfortable clothing, unless your therapist requests differently. If your child is using assistive devices, including bracing, orthotics, hearing aids or other corrective items, be sure to bring them.
