Online patient pre-registration is available for those patients and physician offices with Internet access. If you or your child are scheduled to have any of the below mentioned hospital services provided at the hospital's main campus you may pre-register online.
If you or your child is scheduled for any of the following hospitals services, you may submit your pre-registration here.
- INPATIENT ADMISSION
- OUTPATIENT SURGERY
- OUTPATIENT RADIOLOGY
You cannot pre-register through this website for a physician's office visit or any other physician services.
In order to ensure proper processing, you must pre-register at least 24 hours, or one full day, before your scheduled check-in date.
If you are attempting to pre-register for a hospital service taking place in less than three days, please call the numbers below to be pre-registered immediately over the phone:

To continue pre-registering online, click on the words "Pre-register now!" at the bottom of this page and follow the instructions below. You will be contacted by a registrar ONLY if we need further clarification.
If you have questions about your procedure or your arrival time for a procedure, please speak with your physician.
Report to the hospital on the date that the patient is scheduled for hospital services. If you have any questions regarding your insurance coverage, co-payments or deductibles please contact your Managed Care provider.
You must bring your photo identification and insurance card(s), as well as any paperwork your physician has given you, such as: prescriptions, referrals or authorizations. The registrar will scan these and have you autograph the appropriate forms. At this time, you will also be asked to pay any deductibles, co-payments or unmet out-of-pocket expenses that apply. For your convenience, most major credit cards are accepted.
You will be billed separately by your physician and for any other professional services such as: Radiology (x-ray) interpretations & Pathology (lab) Interpretations that your physician deems necessary, unless otherwise specified.

Please bring your insurance card, a list of all medications, vitamins and herbal supplements your child is currently taking, and your child’s current immunization record.
Do I need to let my insurance company know that my child is going to be in the Hospital? What will they cover?
Always tell your insurance company when your child is going to be admitted to the Hospital. There are so many types of insurance plans; it's hard for us to tell you if you need to get prior approval for your child's Hospital stay. Contact your insurance company or your employer with specific questions about what is - or is not - covered by your plan.
Individual insurance plans vary, but it's always a good idea to contact your insurance company if your child went to the Emergency Room or Urgent Care. This will help to process and pay the claim for services.
It's fine to let your insurance company know that you've had a baby or adopted a child. However, you must notify your employer of the change in dependents. Your employer will then start the formal process of adding dependents to the plan. Most plans allow you to add dependents within 30 days from the date of birth or from the final adoption date.
Coverage varies with each insurance company and employer. Generally, the Hospital staff does not know whether a particular service will be covered. The insurance company that issued your health insurance policy is the best source of information about your coverage. If you are covered under a group policy through work, your employer can tell you what is covered. In all cases, you are the "responsible party" - not your insurer. Therefore you are responsible for all payments for hospital and medical services, even if they are not covered services. Even "medically necessary" services may not always be covered by your insurance contract. Please refer to your member handbook or call your insurance company with questions.
Start by making sure that the treatment facility accepts your insurance plan. Each insurance plan is different. Even within the same company there may be several plans. One plan may cover a certain treatment while another does not. Also, just because Cook Children's Hospital (or one of our facilities) is listed in your plan, it may not be approved for all the services we offer. Contact your specific insurance plan to find out whether your child's treatment will be covered.
Each professional needs to have a separate contract with the insurance companies. The Hospital doesn't know if each professional who cares for your child has a contract with your insurance company. You need to check with your insurance company or your employer about this.
