Please bring your insurance card, your driver’s license and legal proof of guardianship if applicable.

Do I need to let my insurance company know that my child is going to be in the medical center?

It is recommended that you always tell your insurance company when your child is going to receive services from the medical center.

Coverage varies with each insurance company and employer. Each insurance plan is different. Start by making sure that the treatment facility accepts your insurance plan.
