Preparing For Your Visit
Thank you for choosing Cook Children's. A visit or stay in the hospital can be stressful for patients and their families. To help ease your anxiety, we have put together some helpful information and resources to make your arrival to Cook Children's go smoothly.
Starting April 4, Cook Children's Pain Management will serve you in an improved area at the Dodson Specialty Clinics. Please use the new P1 garage to park and check in at the new location of Dodson Central Registration. If you have any questions, just ask us. We're here for you every step! Download map.
Before your visit
Prior to your visit, we recommend that you contact your managed care provider regarding insurance coverage, co-payments or deductibles. Our patient registration representative may contact you with benefit information for your service. Depending on your circumstances, you may consider meeting with someone from our social service department regarding financial assistance.
If your child is under 18 years of age, a parent or legal guardian is required to register them and attend the appointment.
In preparation for your virtual visit, please take some time to complete our online patient and parent pain questionnaires patient history form. This will make it a smoother process.
What you need to bring
Bring the following information to your child's appointment:
- Completed patient history form
- Paperwork for referrals, authorizations and physician's orders
- Your photo identification and insurance card(s)
- You will be asked to pay any deductibles, co-payments or unmet out-of-pocket expenses that apply. For your convenience, most major credit cards are accepted
Upon arrival at the medical center
Please check-in at Patient Registration (map) on the day of your appointment to complete the registration process and receive directions to your appointment location. If this is your first visit, please give yourself plenty of time to find parking and our clinic.
Health Care Notebook
We recognize that caring for a child with pain can be overwhelming with many appointments, medications and recommendations. Our Cook Children's health care notebook can serve as a tool to help you get organized with your child's care, share with other caregivers and health care members and it can be easily be customized to make it your own. We encourage you to print this document and put it in a binder. Bring the binder to clinic appointments or hospital visits to share with any of your child's health care team.
