Services Provided

The services we provide include:

  • Sick and well-child visits
  • Communicate with the Department of Family Protective Services  (DFPS) and child placement agencies
  • Coordinate care between caregivers and primary care physicians, specialists, therapists and behavioral health
  • Help establish a primary care medical home

Program overview

  • Provide a medical screening within three business days for children who come into the Department of Family and Protective Services (DFPS) care
  • Assess the child's overall health
    • Complete medical assessments
    • Complete developmental assessments
    • Complete behavioral screenings
  • Ensure patient continues routine medicines and help with refills and equipment if necessary
  • Discuss and address developmental and behaviorial concerns
  • Connect children to services such as speech, physical and occupational therapy, dental, behavioral health and specialist visits
  • Help caregivers access health care services
  • General connection to education and resources
