Our Fostering Health team has put together helpful information about appointments, answers to questions we are asked most often, prevention and support resources, insurance information, as well as educational resources.
Bring these helpful records to your appointment
We understand that records can be difficult to manage when placing a foster child.
- Bring the following required legal documents:
- Medical Consenter Form 2085-B (Foster Placement) OR Medical Consenter Form 2085-KO (Kinship Placement)
- Placement Authorization Form K-908-2279
Please try to provide any of these before your first appointment:
- Immunization records
- Current medications (with dosage and instructions)
- Any doctors and available records
- A list of previous schools
- Court order establishing Temporary Managing Conservatorship (TMC)
- Safety plan agreement from Child Protective Services
Late arrivals
Please arrive early for your appointment. Late arrivals may have to wait until the next available appointment or reschedule for another day.
Frequently asked questions
Who can be referred to the Fostering Health Program?
There are two different ways a child is eligible for care at our Fostering Health Program.
- Kinship placement or a foster home, through the Department of Family and Protective Services (DFPS)
- Living with biological parents while in DFPS custody
Why can't I access MyCookChildren's patient portal for my foster child?
With the legal and confidential status of children in foster care, patient information is not available for foster care patients.
To report abuse, neglect or exploitation
Please contact the DFPS at 1-800-252-5400. If the child is in immediate danger, call 911.
