Services Provided
The services we provide include:
- Sick and well-child visits
- Communicate with the Department of Family Protective Services (DFPS) and child placement agencies
- Coordinate care between caregivers and primary care physicians, specialists, therapists and behavioral health
- Help establish a primary care medical home
Program overview
- Provide a medical screening within three business days for children who come into the Department of Family and Protective Services (DFPS) care
- Assess the child's overall health
- Complete medical assessments
- Complete developmental assessments
- Complete behavioral screenings
- Ensure patient continues routine medicines and help with refills and equipment if necessary
- Discuss and address developmental and behaviorial concerns
- Connect children to services such as speech, physical and occupational therapy, dental, behavioral health and specialist visits
- Help caregivers access health care services
- General connection to education and resources
