What is the Parent/Caregiver Survey?
Cook Children’s conducts a formal community health needs assessment (CHNA) to help fulfill our system’s Promise – to improve the well-being of every child in our care and our communities. One way we identify the needs of our community for the CHNA is through our Parent/Caregiver survey.
The Parent/Caregiver Survey (formerly known as the Community-wide Children's Health Assessment & Planning Survey (CCHAPS)), available in English and Spanish, gathers statistically representative data of children ages 0–17 living in Collin, Denton, Grayson, Hood, Johnson, Parker, Tarrant and Wise counties.
Cook Children’s administers this survey annually.
Why have I been contacted to participate in this survey?
Our research vendor, ETC Institute, selects a random 25,000+ households who have a child 0-17 years of age and live within the eight-county service area of Collin, Denton, Grayson, Hood, Johnson, Parker, Tarrant and Wise counties to participate in the survey.
How does this affect me and my family?
If ETC Institute contacts you, we encourage you to take the time to complete the survey (about 20 minutes). Parents and caregivers know their children best, and we want to learn about your most pressing challenges and greatest successes. We also value diversity and want to hear from families of different backgrounds. This information helps us develop or improve our programs to address and support unique differences.
