Request Photo or Video Shoot

Throughout the year many companies, organizations, videographers and photographers request to use Cook Children's facilities for photo and video shoots. While we are honored that so many people are interested in using our amazing facilities and patients, we have to make sure each individual shoot upholds Our Promise and that we have the resources available to make it happen.

All outside requests for photos and videos must be submitted by filling out the brief photo request form below. The form must be received at least four weeks in advance. In most cases, we will respond within 48 hours of the submitted request.

Request evaluation

Your request will be evaluated based on the following criteria:

Direct link to the strategic plan of Cook Children's
Potential usage of photo/video(s) for Cook Children's produced publications and websites
Cook Children's mention on the materials created using the photo/video(s)

This criteria helps us evaluate if we can fulfill your requests with our limited resources.
