How to Sign Up for the MyCookChildren's App

In this article, we will be covering the steps needed to create an account for the MyCookChildren's app. Parents and guardians who are wanting to have access to their child or children's health care information with Cook Children's and have not created a MyChart account.

These are the steps needed to sign up for proxy access and allow you to login to the MyCookChildren's app. To add your child(ren) under your access, do the following:

Step 1: download app

Once downloaded to your device, select Sign Up Now. If you have not downloaded the app, here are links to each app store (Apple iOS)(Android). If you prefer signing up on your computer, click here.

Step 2: get started

Select Get Started and select the option that best describes the access you are seeking. Parents and legal guardians will select the appropriate option. Only patients who are 18 years or older can request access to their own information.

Step 3: enter your information

Enter your information. You will be asked if you are a current Cook Children's Health Plan member and if you are a current or former patient of Cook Children's.

Step 4: child's information

Fill out your child's information, including your relationship to the child. You will be asked if your child is a current Cook Children's Health Plan member and if they are a current or former patient of Cook Children's. If you have another child who is a patient at Cook Children's, you can select Add Another Child. This will allow you to add multiple children under the same account.

Step 5: submission and approval

After all the information has been entered, you will be able to submit the completed form. Your completed form will be reviewed and verified by a member of our support team.

Once approved, you will receive an approval notification from the support team within two business days confirming access and will be provided instructions on how to create your login credentials for your account. Those credentials will be used to log into the MyCookChildren's app.
