A new and improved patient portal.

Introducing MyChart, a new and improved patient portal experience for Cook Children's patient families. If you wish to manage your child's health information online, you will need to sign up for a Cook Children's MyChart account.

Please note, if you had an account on the old patient portal, you will need to sign up for a Cook Children’s MyChart account. All the information from the old portal will be accessible in our new portal.

  2. Fill out this form to request an account. Please note, it could take up to two weeks for your account to be activated.
  3. Sign up for an account at any of your child's doctors' offices.

What is MyChart?

MyChart is a new and improved patient portal experience for Cook Children's families. Available in both English and Spanish, it offers convenient online access to your child's medical records so you can spend less time managing your child's health care and more time with your child.

MyChart features include:

  • Accessing summaries from all of your child's visits
  • Receiving test results
  • Viewing upcoming appointments
  • Access and print immunization records
  • Direct communication with your child's care team
  • Refill requests
  • Pay bills online
  • "Track my health" functionality that can be synched with various online health tracking apps

How do I sign up for a MyChart account?

There are three ways to sign up for a MyChart account:

  1. Call 682-303-4367.
  2. Fill out this form to request an account. Please note, it could take up to two weeks for your account to be activated.

What happened to the information in the old patient portal

The majority of your child's information that was contained in the old patient portal will be converted over to MyChart. This includes vaccine records, test results, visit summaries and upcoming appointments.

How much does MyChart cost?

MyChart is a completely free and confidential service that you can access from any device.

What do I need to access MyChart?

All you need is internet access and an updated internet browser to access your secure account.

What if my child has a MyChart account at another facility?

You will still need to sign up for a Cook Children's MyChart account.

Where can I access MyChart?

One of the best things about MyChart is that you can access it anywhere! You can access it online or download the free app in the Google or Apple Play Store.

Why should I sign up for MyChart?

MyChart has many features that greatly improve the patient experience. These include:

  • Accessing summaries from all of your child's visits
  • Receiving test results
  • Viewing upcoming appointments

How is MyChart secure?

Your privacy is our priority. Access to health information is kept private and secure through activation codes, personal usernames, and passwords. All MyChart messaging is done while you are securely logged into the website.

What if my health information on MyChart is incorrect?

This information comes directly from your medical record at your doctor's office. Contact your doctor to correct any inaccurate information at your next appointment.

What do I do if I forgot my password?

Contact our MyChart Patient Support Line at 682-303-4367 to request a new password. There is also a “Forgot Password/Account Reset” link on the sign-in page to reset your password online if that is preferred.

My activation code is not working.

To keep MyChart secure, your activation code expires after 30 days and is no longer valid after the first use. If you are still having problems, call our MyChart Patient Support Line at 682-303-4367.

Where can I update my personal information?

You can update your home address, e-mail, password, etc. by logging into MyChart and from the left menu, go to the preferences section and select the appropriate option.

What is MyChart billing?

Depending on your level of access, MyChart billing gives you the ability to view current open accounts, previous statement information and previous payments, as well as make payments.

Where can I view my billing information?

Once logged into your MyChart account, you can view billing details by clicking the Billing tab which is visible from the Welcome page. Then select “Billing Acct Summary” to access your information.

Can I pay my bills online?

If you have the appropriate level of MyChart access, you can pay your bill online and receive a confirmation number sent directly to your MyChart messaging center.

How do I pay my bill in MyChart?

If you have appropriate access, you can pay your bill on MyChart by logging in and going to your statement. You can find this by:

  • Selecting the billing tab
  • Click the Billing Acct Summary option
  • If you have an outstanding balance, click “Pay” next to the bill(s) you would like to pay

Will I receive notifications for my billing information?

Yes. Notifications and reminders are sent when you make a payment or receive a new statement or billing letter.
