Health care coverage is more than insurance, it's a major investment in your health, and the health of your family. At Cook Children's, we believe that every child should have access to excellent care, and we are working hard every day to ensure that happens.

As of November 09, 2022, Cook Children's Medical Center and Cook Children's Physician Network have contracts with the following health care insurance companies, and we are committed to growing this list. We want to be sure that every family and child in our community has the coverage they need to get and stay healthy.

IMPORTANT:

Cook Children's Medical Center - Prosper

Currently your insurance may be out of network since we are a newly opened facility (Paperwork is still processing with the insurance companies). During this transition we may need to schedule your service at an outside facility or postpone the service for a short time.

If you are seen in our emergency room please note that you may receive paperwork showing that we are out of network from your insurance company. If you have any questions please feel free to reach out to our customer service representatives at 945-204-4100 for assistance.

Questions about coverage

Not all of the plans listed here are accepted by all affiliated companies of Cook Children's Health Care System. If you have questions about coverage for Cook Children's Home Health, or Cook Children's Pediatric Surgery Centers, we recommend that you review their specific insurance plans or contact your health insurance carrier/employee benefits administrator to verify coverage and for information regarding your health insurance plan.

Please be advised Cook Children's DOES NOT ACCEPT ANY Healthcare Sharing Ministry Plans, Physicians Only Product, or Referenced based pricing plans regardless of Plan name- these are SELF PAY.

Physicians may have an existing only or closed panel status with some plans. Please make sure you are aware of your physician's panel status.
