Prepare for a new appointment
Rehabilitation services begin with a referral from your physician. We'll confirm that we've received a referral from your physician before we schedule your appointment. Please contact your insurance company with questions about your benefits. It's your responsibility to understand your insurance plan and benefits. Your insurance company staff can answer coverage questions. Ask insurance company staff about benefits for an outpatient hospital facility.
When you come to your appointment, you'll need to bring:
- Driver's license
- Patient's insurance card
Your first visit
At the first visit, a licensed clinician will test your child as part of the initial evaluation.
Then, using the test results from the first visit, they'll develop a treatment plan. Families help set goals for home programs. The clinician will advise what treatment your child needs.
After the test is finished, your clinician will review the results with you and write a report. Results, goals and a treatment plan will be in the report. Our team will send the report to the referring physician and the insurance company. We'll ask for approval for the treatment plan. After your insurance has approved the treatment plan, we'll call you to make the next appointment.
What happens during appointments?
- Treatment activities are based on your child's treatment plan. We want you to participate in these activities.
- We'll track your child's progress in the medical chart.
- We'll teach you how to do the activities with your child at home.
