An Affiliated Covered Entity (“ACE”) is a group of Covered Entities, Health Care Providers and/or Health Plan under common ownership or control that designates itself as a single entity for purposes of compliance with the Health Insurance Portability and Accountability Act (“HIPAA”). All entities that are part of the Cook Children’s Single Affiliated Covered Entity (“Cook Children’s ACE”) will follow these practices. The Cook Children’s ACE members will share Protected Health Information (“PHI”) with each other for the treatment, payment and health care operations of the Cook Children’s ACE and as permitted by HIPAA and this Notice. As an ACE, Cook Children’s may add or remove Covered Entities as part of the Cook Children’s ACE. A complete list of the Cook Children’s ACE members is provided below. The list will also be made available upon request at any our facilities or by contacting us at 682-885-7822.

  • Cook Children’s Medical Center;
  • Cook Children’s Medical Center - Prosper;
  • Cook Children’s Physician Network;
  • Cook Children’s Home Health; and
  • Child Study Center

Cook Children’s Health Care System provides administrative and operational support services to the entities that are part of the Cook Children’s ACE and may receive or create protected health information on behalf of the Cook Children’s ACE as a business associate.

Pediatrix Medical Group of Texas is part of an Organized Health Care Arrangement with both Cook Children’s Medical Center and Cook Children’s Medical Center – Prosper and has agreed to follow the practices in Cook Children’s Notice of Privacy Practices with respect to the patients its providers engage with at those facilities.
