Conflict of Interest
Effective August 24, 2012, the Public Health Service requires all institutions applying for or receiving PHS research funding to provide public access to certain information regarding financial conflicts of interest held by senior/key personnel research projects funded by the Public Health Service.
Cook Children’s Health Care System will make available the following information regarding financial conflicts of interest identified on and after August 24, 2012 for senior/key personnel on PHS awards:
- Name of senior/key personnel with the financial conflict of interest
- The individual’s title and role in the research project
- The name of the entity in which the financial interest is held
- The nature of the financial interest
- The approximate value of the financial interest (by ranges)
The requested information will be send out within 5 business days of the receipt of the request.
How to make a request
To make a request for information regarding Cook Children’s Health Care System senior/key personnel or a Cook Children’s research project, please send an email request to: CookChildrensResearch@cookchildrens.org. Include FOIA FCOI on the subject line. If information related to a specific senior/key personnel or Public Health Service grant or contract number is being requested, clearly indicate this information in the request.
