If you are an existing patient and would like to schedule an appointment, please call us at 682-885-4260. You can also schedule an appointment through your MyCookChildren's patient portal.
As a reminder, please arrive 30 minutes before for your scheduled appointment time. This will give you time for parking and allow us time to check you in and stay on schedule.

If you're a physician requesting a consult or referring a patient, please download our Nephrology consult/referral request form.
Please fax this form, along with patient pertinent medical records and a copy of the consult/referral form.

Please have the following patient information ready:
- Patient's name and date of birth
- Reason for admission
- Admission type, such as observation or inpatient
- Special needs of patient, such as isolation or equipment
- Patient's medical history
