Our Cook Children's Pain Management Program consists of an Inpatient Pain Consultation Service and Outpatient Pain Management Clinic. In both settings, our pediatric pain team are specialized in the evaluation and treatment of a variety of pain conditions including acute post-operative pain, sickle cell disease, cancer, arthritis, cystic fibrosis, migraines and much more.

Our Cook Children's Pain Management program uses a team approach in caring for infants, children and adolescents with acute and chronic pain. Our program consists of our inpatient pain consultation service and our outpatient Pain Management clinic.
Your visit
Prior to your visit, we recommend that you contact your managed care provider regarding insurance coverage, co-payments or deductibles. Our patient registration representative may contact you with benefit information for your service. Depending on your circumstances, you may consider meeting with someone from our social service department regarding financial assistance.
If your child is under 18 years of age, a parent or legal guardian is required to register them and attend the appointment.
In preparation for your virtual visit, please take some time to complete our online patient and parent pain questionnaires patient history form. This will make it a smoother process.
What you need to bring
Bring the following information to your child's appointment:
- Completed patient history form
- Paperwork for referrals, authorizations and physician's orders
- Your photo identification and insurance card(s)
- You will be asked to pay any deductibles, co-payments or unmet out-of-pocket expenses that apply. For your convenience, most major credit cards are accepted
Upon arrival at the medical center
Please check-in at Patient Registration (map) on the day of your appointment to complete the registration process and receive directions to your appointment location. If this is your first visit, please give yourself plenty of time to find parking and our clinic.
